Primary care was identified with by the employer’s industrial classification codes (SIC2007); 86.211, 86.901, 87.101, 87.102 and specialist care was identified by the code: 86.101. Occupational groups were defined by the 4-level International Standard Classification of Occupation-08 codes; 2211 and 2212 (physicians); 2221–2223 (nurses); 5321 (health professionals); 5329 (personal care workers)).
